Preferred Primary Care Physicians offers two disease specific Care Management programs: Chronic Obstructive Pulmonary Disease (COPD) and Congestive Heart Failure (CHF). The Care Managers provide education on the disease process, assists with care coordination and promote self management with a focus of keeping you healthy, happy, and at home.
