Choosing the Level of Care

Your physician and/or discharge planner at the hospital will offer guidance in determining the most appropriate level of care based on the amount of support and medical care needed. Preferred Primary Care Physicians (PPCP) also provides a medical social worker who will provide information and make the connections that are appropriate.

Once the appropriate level of care is determined a facility or agency must be selected. PPCP has developed collaborative partnerships with select post-acute care providers. These select providers have agreed to work closely with your PPCP physician to ensure a high level of ongoing communication with the PPCP health care team, ensuring that we maintain the continuum of care that is vital to successful, satisfactory outcomes.

Whether the selection is a PPCP partnered provider or another facility or agency, options may depend on individual insurance coverage and/or clinical needs.
